Education:

Bachelor’s Degree in Social Work, Sociology, Psychology or other social services professional field. A minimum of two (2) years experience working with aging/ elderly populations and/or persons with disabilities.

Position Description:

The Aging Services Coordinator reports to the Director of Aging and Disability Services with on-site needs assessments and program evaluation activities as needed. The Aging Services Coordinator’s primary responsibility is to coordinate and deliver high-quality resident services that lead to improved health and wellness among the residents of one or more housing communities for seniors and persons with disabilities. The Aging services Coordinator serves as the Aging and Disabilities Services liaison to residents, on-site leasing staff, community groups, and local service providers and is responsible for the daily operations of the on-site Community Center/s.

Position Responsibilities:

  • Provide community-based care management for residents at assigned community.
  • Provide leadership and support in the assessment of community needs and strengths, as well as the management and evaluation of all service providers and programs.
  • Implement and annual care/ treatment plan to provide services to meet the needs identified through psychosocial assessment as well as provide reassessments to identify changes in service need, health and psychosocial status and determine changes to care/ treatment plan and service provision.
  • Implement a strategic services plan to provide linkages and engage the community strengths identified through the assessment process and observation.
  • Coordinate with the Property Management Teams, calendars of services to ensure efficient utilization of community space.
  • Monitor and ensure compliance of contracts and MOUs with service providers: obtaining proper releases from program participants, maintenance of proper attendance records, progress of participants, and photographic anecdotal evidence of the program’s progress.
  • Assist in the development and implementation of program publicity/marketing strategies to maximize participation in Community events.
  • Train, supervise, schedule and evaluate Community Center volunteers and onsite community organizations as appropriate.
  • Provide input in the preparation of he annual budget. Assist in the management of the Site-specific Aging and Disability Services budget and expenditures.
  • Communicate with Community Partnerships Manger and Property Management Teams to ensure safe operation of equipment and facilities.
  • Other duties as assigned, including but not limited to intake and referral services, outreach, educational program development (health and literacy), economic empower (financial fitness), and emergency assistance.

Position Qualifications:

  • Ability to coordinate, organize and implement services and programs.
  • Ability to select, train, supervises and evaluates volunteer and service provider organizations.
  • Ability to manage multiple priorities and programs within an approved budget.
  • CPR and First Aid certified preferred.
  • Ability to communicate clearly and concisely, both orally and in writing.
  • Ability to prepare and maintain confidential written reports, records and files.
  • Ability to establish and maintain effective working relationships with civic organizations, special interest groups, school officials, City officials, the media, and others.
  • Computer literacy in Microsoft Word, Excel, Microsoft internet Explorer, Microsoft Outlook, Microsoft Publisher, and other relevant software.
  • Experience in establishing and implementing policies and procedures
  • A collaborative, team-oriented work style
  • An enthusiastic attitude with proven ability to organize and coordinate project and work teams.
