RANCHO CUCAMONGA, Calif. – SCAN Health Plan has awarded the Hope through Housing Foundation a $10,000 grant to help its senior residents in emergency situations. One of the nation’s largest not-for-profit Medicare Advantage plans, SCAN is dedicated to keeping seniors healthy and independent.

“Our seniors are so vulnerable to small, unexpected expenses that can become major barriers to their health and well-being,” said Hope through Housing President Greg Bradbard. “With SCAN’s support, this emergency assistance makes funds available to assist seniors in their greatest times of need.”

In partnership with its housing partner, National CORE, the Hope through Housing Foundation invests in the health and stability of low-income affordable housing residents, including emergency assistance and daily supportive services for senior residents in need. The SCAN grant will allow Hope through Housing to establish an Emergency Assistance Fund to provide financial support to older adults (55 and over) who are faced with unexpected expenses. Funds will be used to meet a variety of health and basic needs, including food assistance, housing assistance, transportation, utility payments, adaptive devices, and support for other basic necessities. It will provide important support for Hope through Housing’s Connections to Care initiative, which ensures that senior residents have their basic needs met, are able to maintain their mental health and wellness, and are provided opportunities to connect socially with their community.

Hope through Housing works to utilize affordable housing as a powerful platform for life transformation, empowering residents to reach goals they never previously thought were possible. It works to remove barriers to economic success for vulnerable populations and expand access to economic opportunity in low-income communities by providing a customized mix of services to help affordable housing residents increase their economic mobility and future self-sufficiency.
